Achilles Tendinopathy Treatment
ACHILLES TENDINOPATHY TREATMENT AT RX PHYSIOTHERAPY
At Rx Physiotherapy, we understand how frustrating Achilles pain can be — whether it’s limiting your running, affecting your training, or just making daily movement uncomfortable.
Achilles tendinopathy isn’t just a “tight calf” or something you need to stretch out.
It’s a load-related tendon condition that requires the right balance of loading and recovery to improve.
Our approach is simple:
Reduce your pain, restore your movement, and build your capacity so your tendon can handle load again.
What is Achilles tendinopathy?
Achilles tendinopathy is a condition involving irritation and structural changes within the Achilles tendon, rather than a purely inflammatory problem.
It typically presents as:
- Pain or stiffness in the Achilles (especially in the morning)
- Pain at the start of activity that may ease as you warm up
- Increased symptoms after loading (running, jumping, walking hills)
- Local tenderness or thickening of the tendon
There are two main types:
- Mid-portion Achilles tendinopathy (most common)
- Insertional Achilles tendinopathy (closer to the heel)
Why does it happen?
Achilles tendinopathy develops when the load placed on the tendon exceeds its ability to tolerate it.
Common contributing factors include:
- Sudden increases in running or training volume
- Changes in speed, hills, or intensity
- Reduced calf strength and endurance
- Limited ankle mobility
- Inadequate recovery between sessions
- Previous tendon injury
Importantly:
This is a capacity issue — not just inflammation.
Who We Treat
- Runners with Achilles pain during or after training
- Gym members struggling with calf loading or plyometrics
- Field sport athletes returning to play
- People increasing activity after a period of inactivity
- Anyone with persistent or recurring Achilles pain
From everyday movement to high performance — we guide you back.
Load is the Solution (When Done Properly)
Current best practice strongly supports progressive loading as the primary treatment for Achilles tendinopathy.
This includes:
✔ Gradual strengthening of the calf–Achilles complex
✔ Controlled exposure to load (not complete rest)
✔ Progressive return to energy storage activities (running, jumping)
Complete rest or passive treatment alone is not effective long-term.
Tendons need load to adapt.
Our Approach to Achilles Rehabilitation
At Rx Physiotherapy, we take a structured, individualised approach — no generic protocols, no guesswork.
We start by understanding your load and your goals.
- Thorough assessment of calf, ankle, and lower limb
- Identify whether it’s mid-portion or insertional tendinopathy
- Analyse training loads and contributing factors
- Screen for other causes of posterior ankle pain
- Create a structured rehab plan
You leave your first session with clarity and direction.
Early management focuses on reducing tendon irritability while maintaining movement.
- Modify aggravating activities (not complete rest)
- Introduce isometric or controlled loading for pain management
- Address early strength deficits
- Guide return to tolerable activity levels
This is the most important phase.
- Progressive calf strengthening (key driver of recovery)
- Heavy slow resistance training
- Build single-leg strength and control
- Improve tendon load tolerance
Evidence shows that structured loading programs improve pain and function over time.
Once strength improves, we prepare the tendon for real-world demands.
- Introduce plyometrics and dynamic loading
- Running progression programs
- Sport-specific drills
- Build tolerance to high-load activities
Achilles pain often returns if capacity isn’t maintained.
We focus on:
- Long-term calf strength and conditioning
- Load management strategies
- Training structure and progression
- Ongoing performance support
It’s Not Just the Tendon
Achilles tendinopathy is influenced by the entire lower limb.
We assess and address:
- Calf strength and endurance
- Ankle mobility
- Foot control
- Hip and lower limb mechanics
- Training load and recovery
This is why isolated treatments don’t work —
the whole system needs to be stronger.
Education and Confidence
Understanding your condition is key to recovery.
We help you:
- Understand what’s driving your pain
- Know what level of activity is safe
- Manage symptoms without stopping completely
- Progress confidently back to running and training
YOUR PARTNER IN ACHILLES RECOVERY
At Rx Physiotherapy, we combine:
- Evidence-based treatment

- Performance-focused rehab
- Ongoing support
Because the goal isn’t just to settle your pain —
it’s to build a stronger, more resilient tendon.
Want to Learn More?
Achilles tendinopathy is one of the most common overuse injuries — especially in runners and active individuals.
The good news:
Most cases improve with the right combination of:
Load management, Progressive strengthening, & Time and consistency
The goal isn’t just pain relief — it’s long-term performance and resilience.
